In the continued effort to keep our office safe for patients and employees, LifeThyme requires proof of vaccination or a negative antigen Covid test result the morning of your treatment. If you have any symptoms, we ask that you reschedule.

We have an online schedule at LifeThyme Wellness. Our intake forms are also here, so you can fill them out before your first treatment. The online schedule is easy to use and it will send you an email confirmation and reminder. You can also connect it to your Outlook Calendar, Google Calendar or iCal Calendar so you never forget the appointment. Our online scheduler is available 24 hours a day, 7 days a week – even on holidays and weekends. If you are more traditional and would like to call, that’s great too. Please call 296-5566 and one of the team will get back to you as soon as they can.
If you are a new patient to LifeThyme Wellness please fill out our new patient intake form. Once completed please email it to [email protected] You can also bring it with you when you come for your treatment. There will also be extra forms in the office.
If you are scheduling for an Acupuncture Treatment, you will need to fill out the additional intake form. If you have seem any of the practitioners previously (in the last 3 years) you do not need to fill our the initial intake form again. Please email your acupuncture forms to [email protected]
CANCELLATION POLICY:
-With a cancellation of less than 24 hours, you will be charged a $75.00 fee.
-When you “no show” an appointment, you will be charged 100% of your appt fee.
